Field Trips

 

All student field trips, excursions, or school journeys should have curricular significance.  Prior to a trip, the teacher in charge should establish with students the objectives of the trip so that the students will be guided in their observations and will be able to evaluate and gain from the experience.  If you have any questions regarding field trips or are planning a field trip, teachers should speak to Mr. Fuentes

 

Follow the Field Trip Check List below:

 

1.     Field Trip Request form (Available in the APO)

a.     Destination is on DistrictÕs approved list.

b.     Date of Filed trip does not conflict with other school activity (especially testing dates)

c.     Request form is filled out completely with all necessary information.

d.     Submit form to Mr. Fuentes for approval.

 

2.     After trip is approved, complete form Appendix A and highlighted sections of Application for Auxiliary Transportation/Trip.

a.     Complete items at the top of the above mentioned forms.

b.     Consult with appropriate categorical coordinator (T1 or ELL) to determine funding source for substitutes and for bus funding.

c.     Summit forms to Mr. Fuentes for approval.

 

3.     Complete original of parent permission (trip slip) and permit to leave class forms.

a.     Make copies for students participating

b.     Make a list of students who returned completed forms

c.     Separate emergency info portions of parental permission slip and keep with copy of studentÕs participation list.

 

4.     Make sack lunch arrangements with cafeteria manager at least 2 weeks in advance of trip date.

a.     Collect meal tickets from students the day before trip, and turn them in to the cafeteria with an accurate count of breakfast/lunch needed.

 

5.     On the day of the trip, before leaving:

a.     Send students to the cafeteria to pick up meals

b.     Check students onto bus using student participation list.

c.     Leave a copy of student participation list with top half (non-emergency) of parent permission and permit to leave class form in the Attendance Office

d.     Notify Attendance Office upon your return from trip

e.     Return food coolers to cafeteria.

 

The above items are the sole responsibility of the teacher.
